IMPORTANT PLEASE READ
Please read the medical questionnaire carefully as under diving regulations we are not allowed to accept amended questionnaires if you make a mistake or allow you to fill out a second one. If you need to answer ‘yes’ to any of the conditions marked with an*, you will need to bring a medical certificate/letter from a doctor, stating you are ‘fit to dive’ before we can take you into the water. If you can answer ‘no’ to all the conditions, then no certificate/letter is required. If you have a medical showing you are ‘fit to dive’ that is great, but it must be dated with 12 months of diving to be valid.
Don’t worry if you are unable to complete the form before you arrive, we have them here for you. We just like to offer it in advance in case of any ‘yes’ answers, to give you an opportunity to see a doctor if necessary before you arrive, rather than spend any of your precious holiday time sorting this out once you are here.
We can also recommend a doctor locally that can provide a medical for as little as €30 if you’re unable to get a medical before you arrive.
